Diplopia: a complication of dural puncture.

چکیده
DIPLOPIA or extraocular muscle paralysis (EOMP) after dural puncture has been reported occasionally, primarily in the neurology and ophthalmology literature. Because there seems to be a window period before diplopia manifests after dural puncture, the patient and physician may not always believe that the symptom is secondary to dural puncture, particularly when it occurs after resolution of a post–dural puncture headache (PDPH). Diplopia can be so disturbing that it may prompt the patient to seek immediate medical attention. Many patients have been referred to neurologists or ophthalmologists for extensive investigation. Therefore, it is not surprising that anesthesiologists may be unaware of this complication. The first case of diplopia after dural puncture was reported by Quincke more than 100 yr ago. Nonetheless, the latest major anesthesia textbooks describe little, if any, of this complication. It is important that anesthesiologists, emergency physicians, neurologists, and ophthalmologists recognize and communicate findings related to this distressing complication.

Bilateral sixth cranial nerve palsy after unintentional dural puncture.
PURPOSE Bilateral sixth nerve palsy is a known though uncommon complication following dural puncture. The recommended treatment consists of hydration and alternate monocular occlusion. The value and the timing of an epidural blood patch (EBP) for sixth nerve palsy remains controversial as some authors have demonstrated benefits in performing an EBP early in course of the nerve palsy whereas oth...
متن کاملPost spinal puncture headache, an old problem and new concepts: review of articles about predisposing factors
Post spinal puncture headache (PSPH) is a well known complication of spinal anesthesia. It occurs after spinal anesthesia induction due to dural and arachnoid puncture and has a significant effect on the patient’s postoperative well being. This manuscript is based on an observational study that runs on Babol University of Medical Sciences and review of literatures about current concepts about t...
متن کاملEffect of prophylactic gabapentin on post-dural puncture headache in patients after cesarean section surgery
Introduction: Post-dural puncture headache (PDPH) is the most common complication of spinal anesthesia. Numerous drugs have been proposed to prevent or treat post spinal headache, but no definitive treatment is still available. Gabapentin has widely been used for prophylaxis or treatment of different types of headache, so in this study, the preventive effect of gabapentin on PDPH after cesarean...
متن کاملAn alternative management procedure after inadvertent dural puncture
BACKGROUND Accidental dural puncture during epidural blockade is a rare complication. Since postdural puncture headache (PDPH) is the most frequent complication, a wisely management method will also reduce the incidence of PDPH. METHODS Five patients who had inadvertent dural puncture during the epidural procedure before hip or knee arthroplasty were included in this study. After dural punctu...

CASE REPORT Two cases are presented in which the complication of dural puncture is documented in the context of a lumbar transforaminal epidural steroid injection. The hazard of dural puncture during transforaminal epidural injections, the anatomy of the dural and thecal sac, the potential for subdural injections, and relevant literature are reviewed. DESIGN Report of two cases. BACKGROUND ...
